East Jordan Family Health Center has been providing quality health care services since 1976, when the community raised funds to bring a physician to town.

Since then the community Board of Directors has guided the health center from a one physician office to a multi-site specialty clinic.  Providing a full range of primary and preventative services.

Need a little help?

We believe that everyone has the right to affordable health care.  The health center has developed a Sliding Fee Discount Program for patients and their families based on income and family size.  This program may provide you with a substantial discount on the following services:  on-site office visits, on-site procedures, on-site X-ray services, and assistance with prescription costs at approved pharmacies.  

The health center also participates with the Michigan MI Child Program, Community Choice Michigan, and other Medicaid qualified health plans to provide patients with quality health care.
