Site Description: East Jordan Family Health Center is a nonprofit corporation and a federally qualified health center, founded in 1977. 34,000 visits were provided to 10,000 medical users in 2008. EJFHC utilizes a healthcare team approach with physicians and mid-level providers supported by clinical and clerical staff. Satellite offices in Bellaire and Central Lake support the healthcare needs of nearby rural communities. Collaboration with Charlevoix Area Hospital and Northern Michigan Hospital allow for a hospitalist arrangement for inpatient needs. Onsite services include digital X-ray and mammography, laboratory, dental, counseling, and optometric services. In 2009, our health centers successfully implemented Next Gen Electronic Practice Management and Electronic Medical Records. In June 2010 we received certification as a Patient Centered Medical Home.
